ABOUT THE PRACTICE:

Our practice is unique in that when you come to our office for your first examination we sit down with you in an interview office to take a full medical history and listen to your concerns and needs. Then we proceed to the examination room for your exam. Our philosophy is Taking the Time to Listen to our clients which gives us a better picture of the whole person when trying to make a diagnosis.

Dr. Martin J. Weiner established his solo practice in 1988 in Gurnee Illinois. With the addition of associate practitioners, in 1990 the practice was re-named Associates For Women's Health.

Our Gurnee location has remained the same except for a suite change in 1998 due to growth. We have an additional office location in Waukegan. Our office staff is fluent in Spanish.

APPOINTMENTS

All of our patients are seen by appointment. Knowing your time and ours is valuable, we strive to adhere to a schedule. Emergencies and obstetrical deliveries may delay the physician on occasion. If the physician is extremely behind schedule our office staff will offer to reschedule your appointment. Tardiness to your appointment greater than 15 minutes may result in being seen as time permits, seeing another practitioner in the office or appointment rescheduling.

Monday through Thursday 8am to 4:30 pm
Friday 8am to 1:00pm
Saturday 8am to 11am

Waukegan
Tuesday and Friday 9am to 4pm

Appointments for New Patients:
When arriving to our office for the first time you should plan to arrive 15 minutes before your appointment so that you have adequate time to complete history information and insurance forms. Remember to bring in your insurance card and photo identification card. If you don't have insurance ask the receptionist for an estimated cost for your visit. Payment is expected at time of service. HMO and PPO insurances usually have copays to satisfy. We except personal checks, visa, mastercard or cash.

Notifying us of a cancellation 24 hours or more in advance is appreciated so that we may accommodate other patients to our schedule.

When arriving to the office:
-Inform the receptionist of your arrival and sign in
-Update any changes in your information that we have

-For annual exams complete a medical history update form

 

INSURANCE
We accept most insurances When calling to make your appointment have your insurance card accessible so that you can give the receptionist your insurance information. With HMO insurance it is important to read on the card the IPA site of your WPHCP(Women's Principal Health Care Provider) so that we can tell you if we participate in that IPA.
If you don't have insurance ask the receptionist for an estimated cost for your anticipated visit and let the practitioner know that you don't carry insurance. Payment is expected at time of service unless prior arrangements have been made.

 

TELEPHONE CALLS AND MEDICATION REFILLS

-Medication refills- We don't want you to run out of your medication. Please remember to call during office hours before you are completely out of your medication. When you call have the medication name, the pharmacy name and telephone number, and leave a number where you can be reached if we have questions. Your prescription will be called in, within 48 hours. Some medications will not be refilled without an examination or consultation.

-Telephone calls-When you call into the office with an emergency someone will talk with you at that time. If no medical staff are present we advise you ask the receptionist to page the physician on call or dail 911. If it is after hours and you have an emergency go to the emergency room and the physician on call will be notified.
-If you call with symptoms of an infection we attempt to call you by the end of the day.
-If you call with general questions, wanting test results we will call you back within 24-72 hours. Pap smear results take 3 weeks to return to the office from the date the sample was taken.

-When leaving messages make sure you leave your day and evening phone numbers where you can be reached.
